Description:
The AFHS 4Ms framework is an evidence-based, standardized approach to address the health and social equity needs of our older adult patients, especially those with complex care needs. Care strategies focus on identifying what matters most to patients and families, then applying these goals to medication management, mobility promotion, and support for dementia, delirium, and depression (mentation) prevention and early intervention. This presentation evaluates the importance and impact of Age-Friendly care through a case study and then reviews national policy changes and promotors of the 4 M's framework adoption. Additionally, the presentation will provide an overview of how the Age-Friendly framework promotes equitable care and aligns with Sutter's mission and values.

Learning Objectives
After Attending this Activity, Learners Should be Able to:
1. Identify how older adults are at risk for disproportionate harm in healthcare encounters and the impact this has on health equity.
2. Discuss the individual components and inter-relationship of the 4 M's framework: “What Matters” to the patient and family, “Medications,” “Mentation,” and “Mobility".
3. Evaluate the impact of incorporating the 4Ms assessment and care strategies in the outpatient and inpatient setting through a case-study.
4. Describe current and anticipated national policy changes related to the 4Ms as a driver of high-quality, equitable care for older adults.
5. Apply Age-Friendly Health System care practices at local institutions, both internal and external to Sutter, to promote collaboration towards Age-Friendly communities.
