Course Description
When a patient’s health starts to decline, difficult conversations and decisions should be had between them, their loved ones and health care providers to ensure every outcome is patient-centered. This session focuses on strategies for educating the patient and family about one’s health and helping them understand their options in addressing the plan of care and the expected outcomes of each of those options. It is also essential to understand one’s treatment preferences along with their goals for their care so that care decisions are individualized to their personal values.

• Recognize the importance of honest, open communication when addressing a person's declining health.
• Discuss the anatomy of a family meeting and how to give bad news.
• Utilize patient-centered, goal-directed conversations and strategies to address requests, conflicts, differing expectations and prognosis questions.
