Course Description

Sutter's Clinical Quality Coding team's clinician chart reviews identified documentation and coding errors regarding active vs history of cancer medical conditions. Accurate documentation and coding, aligned with CMS documentation and coding guidelines for Active vs History of Cancer, ensures that the patient's medical record accurately reflects the patient's full burden of illness to all members of their care team. This course is designed to provide documentation guidelines and key strategies to help identify when to code active vs history of cancer to our Sutter Physicians and APCs, thus allowing for optimized care plan development.

Learning Objectives

After completing this course, learners should be able to:

1. Discuss correct application of CMS documentation and coding guidelines for Active vs History of Cancer.

2. Demonstrate competency in the application of CMS documentation and coding guidelines for Active vs History of Cancer to significantly reduce or eliminate the number of diagnostic codes which need to be changed.
