SCAH_Wound, Ostomy and Continence Competency Education_Live - August 23rd Classes

Clinicians often lack the necessary skills and knowledge to effectively manage patients with a stapled surgical incisions, nephrostomy tubes, or fecal incontinence. The gap consists of assessment, documentation, wound care, staple removal and supply management. The Lead Wound, Ostomy, Continence Team will provide education with the goal to increase competency in the following: Surgical Incisions and staple removal, pressure injury prevention in home health & hospice, nephrostomy and other tube and site care, and skin care for patients with fecal Incontinence.

Learning Objectives

After completion of this session, learners should be able to:

1. Demonstrate care using evidence based practice and patient centered care for those individuals with staples surgical wounds.

2. Utilize appropriate supply management for surgical incisions.

3. Demonstrate a comprehensive understanding of social determinants and implicit bias related to pressure injury prevention.

4. Apply new recommendations and techniques for assessing and preventing pressure injuries in patients with darker skin tones.

5. Effectively address frequently asked questions regarding the ordering process and qualifications for DME.

6. Analyze and utilize the attributes of the Braden Risk Assessment Tool to accurately assess pressure injury risks in patients.

7. Demonstrate competency in providing effective site care for nephrostomy, biliary, and wound drainage tubes, ensuring proper aseptic techniques, and appropriate dressing changes.

8. Recognize and promptly address skin complications related to the presence of tubes, including identifying signs of infection, skin breakdown, and irritation, and implementing appropriate interventions and wound care strategies.

9. Effectively manage and address potential complications associated with nephrostomy, biliary, and wound drainage tubes, such as blockages, dislodgement, and leakage, by employing appropriate troubleshooting techniques and interventions.

10. Summarize the physiological aspects, common causes, and symptoms of fecal incontinence, as well as its impact on patients' physical and psychosocial well-being.

11. Describe various management techniques to prevent complications associated with fecal incontinence.

12. Summarize steps in using, maintaining, and troubleshooting bowel management systems as well as in instructing patients and caregivers on its use.

13. Develop and implement individualized education plans for patients and caregivers, addressing the specifics of fecal incontinence and its management. They should be able to provide compassionate care and support for patients and caregivers, helping them cope with the emotional aspects of the condition.
