0640_SHVGR_Hospital Management of Acute Heart Failure_Live

April 9, 2025

Overview

Description:

The management of acute heart failure in hospitals is especially important because it requires rapid assessment and intervention to prevent organ damage and reduce mortality. Effective hospital protocols including prompt diagnosis, careful fluid management, appropriate medication administration, and continuous monitoring can significantly improve patient outcomes and reduce both hospital length of stay and readmission rates. This presentation will provide the target audience with the knowledge and strategies necessary to provide best-practice, evidence-based care to patients admitted to the hospital with acute heart failure.

Target Audience

Nurse Practitioner (NP)

Nurse, Registered (RN)

2) Recognize inpatient pharmacists' role during hospitalization and transition to outpatient care.

3) Apply heart failure clinical care pathway tools to manage hospitalized patients. 4) Identify best practices to reduce rehospitalization risk.
