This event encompasses the conclusion of a system-wide program to enhance Sutter’s patient throughput and care transitions strategies through various acute hospital channels that have been collectively identified as the Care Progression work. This work involved leaders and frontline staff from all departments that are involved with the patient care pathway. This presentation focuses on standardization of care progression processes and to support sustainability measures for the updated practice framework implemented across Sutter sites to preserve quality improvements achieved over the last 6 months. Care Management Capstone Day will review 2023 successes such as reduced hospital readmission rates, patient length of stay, and avoidable delays in different subcategories. It will also cover the Care Progression Playbook which encompasses directions, tools, and general guidance for Sutter leaders to implement processes that better support the delivery of patient care and improve the patient experience.

Learning Objectives
1. Review Sutter specific data associated with care management processes such as ELT data, hospital readmission rates, patient stay duration, and avoidable delays in care.
2. Integrate standard care progression processes and leading practice frameworks outlined in the Care Progression Playbook into care management. Review ELT data from 2023.
