The United States is experiencing a demographic shift characterized by an aging population and increasing diversity. As of 2020, approximately one in six individuals in the U.S. is aged 65 years or older and close to 11,000 more turn 65 every day. With population aging, healthcare needs often become more intricate and yet, health systems frequently struggle to manage this complexity effectively. This inadequacy can lead to a disproportionate level of harm among older adults within the healthcare system. To address these challenges, Sutter Health is dedicated to becoming recognized as a "Committed to Care Excellence" Age-Friendly Health System (AFHS) by Winter 2025. To support this initiative, this course is designed to educate future AFHS champions on the essentials of this evidence-based framework and the foundational principles known as the 4Ms: What Matters, Medication, Mentation, and Mobility. The 4Ms framework provides a structured approach to delivering comprehensive care to older adults, ensuring that their needs are met across all interactions with the health system.

Learning Objectives
By the end of this activity, learners will be able to:
1. Summarize the essential components, or “4Ms”, of IHI’s Age-Friendly Health Systems (AFHS).
2. Differentiate between the 4Ms of AFHS and the 5Ms of Age-Friendly care as required by CMS (for acute care).
3. Consider the “why” and “why now” of Age-Friendly quality improvement at Sutter Health.
4. Describe how the Age-Friendly implementation will occur at Sutter Health, using the ADKAR model of change management and implementation science theory.
5. Identify the role responsibilities of individuals involved using the DARE model of decision making.
6. Support the readiness of your team, using Sutter created tools based on IHI and CMS identified drivers of effective and sustainable change.
