The Mentation "M" of the Institute for Healthcare Improvement's Age-Friendly Health System (AFHS) framework focuses on preventing, identifying, treating, and managing dementia, depression, and delirium across care settings to ensure every older adult receives the best care possible, is not harmed by care, and received goal concordant care aligned with "what matters most to them. This activity is designed to support home health clinicians in recognizing the difference between delirium, dementia, and depression. The presentation will especially focus on delirium, which goes unrecognized and untreated in more than 50% of cases, to support home health clinicians develop competencies in evidence-based assessment and intervention strategies to address both new-onset and persistent delirium in home health settings.

Learning Objectives
After completion of this activity, learners will be able to:
1. Prioritize standardizing Age-Friendly approaches to screen early, regularly and specifically for dementia, depression, and delirium in home health settings.
2. Demonstrate accurate screening for delirium in home health settings using the CAM tool embedded in OASIS-E documentation.
3. Evaluate need for further evaluation with new onset delirium.
4. Recommend to patients and family's strategies to manage manifestations of persistent delirium.
5. Collaborate on novel and innovative strategies to develop, implement, evaluate and sustain Age-Friendly health system delirium reduction efforts across the care continuum.
