Every day, health care providers counsel patients around how to lead healthier lives in order to prevent disease. We often do this without understanding a person’s true risk, or their social drivers of health. The World Health Organization defines social drivers of health as “the non-medical factors that influence health outcomes.” Examples of social drivers of health are race, preferred language, sexual orientation, gender identity, access to stable housing, healthy food, reliable transportation, ability to pay for utilities, and personal safety. Although very important, there is complexity to asking about social drivers of health. Who should ask, and when? How should that information be recorded? When someone identifies a risk and/or a need, what resources are available? There are evolving regulatory requirements around screening for and addressing social drivers of health, and health care providers must be prepared. This presentation will focus on the latest requirements for data collection on social drivers of health (SDOH), sexual orientation and gender identity and race, ethnicity, ancestry and language (REAL). She’ll offer suggestions for operational plans to meet these requirements and how to better serve patients.
