Description:
Patients with Heart Failure with an associated Reduced Ejection Fraction (HFrEF) are too often not managed using guideline-directed medical therapy or managed by HF specialist in cardiology. This presentation will focus on best practice and evidence-based care interventions that can help optimize clinical outcomes and quality of life for patients living with heart failure. The presentation will additionally highlight key tools that can be incorporated into day-to-day management of heart failure patients as part of a Heart Failure Clinical Care Pathway.

Learning Objectives
After attending these sessions, learners should be able to:
• Discuss best practice, evidence-based care interventions that have been shown to improve clinical outcomes and quality of life for patients living with heart failure.
• Describe the role of guideline-directed medical therapy (GDMT) in reducing mortality and hospitalizations among patients with heart failure and reduced ejection fraction.
• Apply the key tools developed as part of the Heart Failure Clinical Care Pathway in the day-to-day management of heart failure patients.
