0185_SHSO_ Delirium Across the Continuum of Care_Live
Delirium is an altered state of consciousness characterized by confusion, disorientation, agitation and sometimes hallucinations. Persistent delirium can last for weeks or even months if not addressed promptly, yet patients with delirium are subject to missed diagnosis, misdiagnosis, and missed opportunities for early intervention. These risks of harm can be further amplified when care provided is inconsistent across the continuum of care, impairing both short- and long-term outcomes and increasing risk for readmission. This presentation provides strategies to address these concerns from a medical director of Home Health and a Nurse Practitioner implementing an Age Friendly framework in acute care settings.

1. Describe the components of an Age Friendly Health System.
2. Discuss support resources, including mentorship opportunities, available within Sutter Health to assist other locations in advancement on the journey towards achieving Age Friendly designation.

4. Redesign health education materials using health literacy principles.
5. Utilize shared decision-making to address discharge preparations early and ensure that patients have the ability to get to follow-up appointments with physicians.
