Course Description:
Often times, Patients and providers have difficulty trying to transfer care from pediatric to adult focused care. This has led to interruptions in health care, higher ED visits and lower experienced quality of care. This grand rounds is unique in a way that it targets both pediatric and adult medicine clinicians with a focus on the smooth transition between pediatrics to adult care and options providers can suggest for our teenager patients.

Learning Objectives
1. Discuss some of key barriers to transitioning care from pediatrics to adult care teams.
2. Apply strategies to effectively support any adolescent making these care transitions.
3. Apply strategies to effectively support special populations (children with special health care needs) as they transition and transfer to adult services.
