Course Description
The Patient-Centered Home Care Experience Seminar Series is designed to support the healthcare team in improved communication with their patients, address their role as a leader in their practice, give them tools for communicating with and understanding patients from diverse backgrounds, develop skills in safety for both the patient and the healthcare provider, understand patient’s needs related to their diagnosis and support the relationship between the patient and the healthcare team across SCAH. This first session of the 10-part series focuses on meeting patient and family needs for an individual with Neuro-Cognitive Impairment.

Learning Objectives
After attending this activity learners should be able to:
1. Summarize common symptoms of dementia and apply evidence-based best practices in the care for a patient with dementia.
2. Identify and communicate a team focus to promote individual patient safety.
