Session Description:
2024 SCAH Education Series: Disease Management in Home Health and Hospice Seminar Series is designed to support the healthcare team in improved understanding of their patients' conditions, address their role as a leader in their practice, give them tools for communicating with and understanding patients' unique needs, understand patient’s needs related to their diagnosis and support the relationship between the patient and the healthcare team across SCAH. The series will be delivered in a live zoom format and will have an interactive experience for attendees to ask questions of the presenter.

Session Objectives:
- Have an understanding of Diabetes Mellitus pathophysiology and presentation
- Understand lifestyle modifications for individuals with Diabetes.
- Understand common Diabetes Medications for use, side effects and patient education.
