Deborah Wolff Baker, MSN, ACHPN, FNP-BC, GS-C
Deborah has been an RN for 40 years and a Nurse Practitioner since 2008. She is Certified in Family Practice as well as in Advanced Practice Hospice and Palliative Care. She was an item-writer for the GNC Gerontological Specialist Certification Exam and holds the GS-C certification. She has been a House Calls model developer and leader in the field of Home-Based Primary Care, and currently works with Dr. Ordona as the Sonoma County Annex of his Senior Care Clinic House Calls practice.
Her multiple State and National leadership roles have included three terms as President of NCCGAPNA, Chairperson of the GAPNA National House Calls SIG, Board Member of the American Academy of Home Care Medicine, Board Member of the California Association of Long Term Care Medicine and a past voting Delegate to AMDA-PALTCM, Chairperson of the GAPNA National Health Affairs Committee, and, she is currently Chairperson and convenor of the GAPNA-led Home Health and Hospice Coalition
Deborah has written and published twice in the Journal of Geriatric Nursing and frequently speaks at State-wide and National Conferences on the topics of Home-Based Primary Care and Health Policy. She has a passion for improving care of older adults and works to help shape policies designed to improve their health and Quality of Life by advocating for improved models of Community Based Services through advancing APRN Scope of Practice.
