Description:
Heart failure is a leading cause of morbidity and mortality across the world. Despite its use in outpatient care settings, the use of guideline-directed medical therapy (GDMT) for in-hospital initiation and post-discharge titration by pharmacists working in the acute care setting remains suboptimal in chronic heart failure patients with close to 20% of patients being re-hospitalized and nearly 10% of patients perishing from the condition. In-hospital GDMT initiation and titration has a rapidly expanding body of evidence to support its efficacy and safety. This course serves to bridge the gap in knowledge of GDMT, the medications therein, and the sequencing of administration.

• Evaluate the evidence associated with initiation and maintenance of GDMT during heart failure hospitalization.
• Recommend approaches for GDMT optimization tailored to individual patient clinical scenarios.
• Explain why inpatient and TOC pharmacists are essential for GDMT optimization during heart failure hospitalization.
