2007 Heart Failure Continuum of Care Project to develop a continuum of care eliminating the gaps in care between inpatient & outpatient care for heart failure patients. Model of care piloted as a partnership between Alta Bates Summit Medical Center and Sutter Care at Home. Model development was supported by a Gordon & Betty Moore Foundation Grant.
2006 Implemented the use of Tablet PC’s used by the heart failure program for data collection, data management, and patient follow-up. Together with Sutter Health Information Systems designed and developed data collection tools (MiForms), data identification, and APOLLO template design for the inpatient and outpatient heart failure program.
2006 APOLLO Heart Failure telemangement software development and implementation in conjunction with the Sutter Information Systems department for use at MPHS and other Sutter facilities.
2005 - 2006 Developed, implemented, and coordinated Mills-Peninsula Health Services the “Make it HAPPEN” program with adolescent children and their families. Make it HAPPEN is a program for children who are overweight to stabilize their weight by increasing their activity/exercise and teaching to eat a healthier diet. The “mind, body, spirit” approach was also used with this program.
2004 - 2006 Developed, implemented, and coordinated Mills-Peninsula Health Services Metabolic Program. An “mind, body, spirit” program bringing together traditional lecture series, behavior modification through group work, and using meditation, yoga, tai chi and other modalities to reduce stress and disease progression.
2003 Mills-Peninsula Health Services and Sutter Health Women’s Heart Advantage project to increase awareness of heart disease in women. Have assisted in developing educational materials and given presentations to staff and the community regarding women and heart disease.
2002 Development of an inpatient Diabetes management program and a telemangement program at Mills-Peninsula Health Services.
2001 Developed the Medical History & Medication Log booklet to be given to patients at Mills-Peninsula Health Services as they access care in & outpatient areas.
